GP Practice Based Commissioning

The Lib-Dem; Conservative government has published its proposals for the NHS in their coalition agreement. A central theme is to enable GPs to commission care on behalf of their patients. This will give GPs much greater influence over what is commissioned in their locality, including their ability to manage “hard” budgets. Harmoni has considerable experience in working with GPs on both commissioning and delivering services. Harmoni will work with GPs and commissioners to design and then robustly manage patient demand in healthcare services using an Integrated Care or HMO model. This can be either as a GP Joint Venture or with a GP consortia within a locality of between 50,000-300,000 patients This proposed service is driven by the need to treat patients in the community, wherever clinically appropriate and will be achieved by horizontal integration. The Harmoni Integrated Care Hub (ICH) , has a variety of demand management tools  embedded in its core and its heartbeat is a 24/7 contact centre which will manage all unscheduled and scheduled referrals,  including Out of Hours.
