Harmoni believes that, in the ever-changing landscape of the Health Economy, it is imperative that an holistic approach is adopted by PCT’s to reduce demand at the hospitals by re-focussing and re-engineering their provider services. In parallel with such an initiative it is vital to create an effective referrals process which will ensue that appropriate care is given to the patient, in the appropriate environment. It is axiomatic that this care must be cost-effective and clinically safe. Harmoni also envisages a sustainable and multi-faceted Unscheduled Care solution which interacts across General Practice, through Intermediate Care services, across GP out of hours and social care. Harmoni consider that it is vital to engage the multitude of stakeholders in the turnaround so that they can fulfil their roles effectively to ensure the success of the plan.


In the modern world, patients must be able to access healthcare quickly, effectively and safely. The majority of patients intuitively know the best service to meet their needs. However, an unacceptably high percentage of patients are referred to an inappropriate service such as A&E attendance, prolonging their discomfort and invariably costing the NHS more.


Harmoni is already supporting a number of GP consortia to develop and deliver services under PBC.  We have identified this as a vitally important part of our future business. It is not Harmoni’s intention to support small scale initiatives which maintain a status quo that, in many areas is cumbersome and ineffective.

 

With a mixture of IT and modern business practices, Harmoni has fully embraced the delivery of new services under PBC. We wish to work with PCT’s and GP consortia to fundamentally change the way local healthcare is delivered. We believe that such a partnership will be an irresistible force for change which will benefit the patient and the overall health economy, but will always be free at the point of delivery. Harmoni beleives that Demand can be effectively managed by using:

 

A Single Point of Access (SPA) for all unscheduled referrals
A Scheduled Pathway Management Service (Quartet) for all scheduled referrals

A Long Term Condition Management Facility (LTCMF)
